Background

The patient was a 75yo male with a history pericardiocentesis at a large tertiary cardiac center 3 weeks prior to this admission. He presented with increasing shortness of breath and the emergency department team expected that the pericardial effusion had returned. The echocardiographic findings were quite surprising.
